DC Chartered Health to provide customized assistance to people with HIV
The latest New WASHINGTON, DC – DC Chartered Health Plan is the first managed care organization participating in the positive development of AIDS Partnership Washington Road. This new initiative in the District of Columbia HIV assists African Americans in the districts 5.8 to participate in HIV care and support services safety net, with a particular focus on women and the ways it uses partners.Positive network of trained community members – community health workers – who help their peers to improve their health. Positive Pathways is run by the Washington Association of AIDS, an initiative of the Washington Regional Donor, in collaboration with more than 15 members of the community and the DC Department of Health. CommonHealth ACTION Institute for Innovation in Public Health serves as the primary coordinating agency in close collaboration with the Washington AIDS Partnership and community organizations working together to implement and evaluate all effort.Chartered is a community of peers trained health workers co-located with the case and disease management staff to complement the dissemination strategies to members who are HIV positive. The health worker Community participated in a course of 8 months of training health workers in the Community College in Washington DC. The outreach effort will focus on building trust, inform participants about living with HIV and personalized assistance to help navigate through the medical delivery system. “This program is a small down payment on our efforts to improve our support to members through innovative partnerships with community-based organizations. Our approach is to listen to our partners, suppliers and community partners to share best practices so that we can make the best of what we do with the expertise of our partners, “said Karen Dale Executive Chartered. “These health care workers of the Community offer the confidence based on information and education.
